Where have your failures been with your doubles?

I would submit that double rifles are just about foolproof but any working mechanism can have issues. Break a spring, break an internal lock, get sand in the action chamber, etc.

While i am sure that 99% of thr time, your doubles never have an issue. My question is on the rare occassion where it has happened, what happened and how did you address the issue?
 
The only issue I have ever had with the 8 DRs that I have owned was with my Merkle. The sear that prevents double firing broke. In my follow up research I found while this is a great feature of the brand, the sear breaks quite often. I immediately sent it to JJ Perodeau who easily fixed it.
 
My only problem was a broken stock at the wrist on a between the wars german double. It is currently out being restocked.
 
My WW1 vintage .475NE cracked in the wrist after more than 100 years.. Gun oil seeping into the wood over the years and subsequently rotting it. Had to restock her.. Beefed up the grip..
