Traveling with a minor child

I am going over with a group in June and one member is bringing her daughter without the other parent. I have read conflicting things about South Africa's requirements of a minor child traveling with one parent and if a consent letter is needed and their government form. It would be nice to try and sort out to avoid an issues.

Has anyone had to address this issue in the past year and if so what are the requirements.

I traveled with a minor in the last 6 months ( me and daughter, mother stayed) with no issues , if minor holds US pasport and same last name , they good. I did take birth certificate with but no one asked for anything going or coming.The issue is from SA to other countries( living in SA) from past experience
