S.A. Covid-19 Travel Health Questionnaire - Discontinued or New Address?

I'm trying to open the above link to the required S.A. health department questionnaire. Required to be completed online not more than 48 hours before start of travel. (Or so says my notes.)

I keep getting the dreaded:

This site can’t be reached​

Any ideas on what is going on? This has been over several days and searches on the S.A. Department of Health website produce nothing.

I had the same ‘dead link‘ response over the weekend & went to my go-to site for the latest Country COVID Travel Requirements: IATA.org.

International Air Transport Association (IATA) has a vested interest in keeping Air Traveller informed & ‘flying’. It appears S.A. took the online Hlth Questionnaire site down & we’ll be completing an Ole Skool paper product @ Joburg. Works for me …

You can fill the form out on the plane prior to landing as they will hand them out. I did it electronically upon arrival but it takes just a couple minutes to fill out the arrival or departure forms. They don’t even look at them, they take the form from you and take your temperature and on you go.

As @Jaegger mentions above, bring a few cheap hotel pens! I’ve had the Visa officials in Zimbabwe keep my pens as they didn’t have any, more than once!
 
Gents: I located & included the RSA Traveller Health Questionnaire - "Entry Screening", this version may be what the Incoming Traveller will require.
