As far as I am aware no visa is necessary for U.S. citizens. The arrivals/departure form is simples. You will either be handed one on the plane prior to arrival or pick one up in the arrivals hall.

Agreed, I‘m equally U.S. Citizen & have hunted Namibia 5X, 2007 - 2019 & no pre application Visa required.
If you travel w/ Rifle - complete your pre application rifle permit online & email approx 1-week before arrival … any earlier, they will misplace & blame you! I know …
